Covered CA Enrollment Clinic (Only 3 Sessions Left)

Join us at the Union City Library on Tuesdays (through March 25) from 4pm-7pm for an enrollment clinic about health insurance for you or your family. Enrollment Specialists, Certified Educators and Enrollment Agents will be there to answer questions about health insurance and enrollment on site.

Please bring the following to the enrollment clinic:
 Identification
 Social security numbers
 Proof of citizenship
 Proof of residency -bill, lease or rental agreement
 Proof of Income-Pay stub, taxes, self certify letter
